Long-Term Condition Health Diary

An editable private observation record without automated interpretation.

Record observations, not diagnoses

Keep concise notes on symptoms, appetite, weight, medicines, episodes and appointments. Include test results only as supplied and explained by the veterinarian. Add units and dates to avoid misleading comparisons. A diary supports conversations; it must not delay urgent contact.

Agree what is useful

Ask the team what to track and how often. A condition-specific plan may be more useful than recording everything. Note changes from the dog’s baseline and what the practice advised. Keep copies privately and share with authorised carers or the veterinary team.

Avoid access codes, bank details and unnecessary personal health details.
