Owner resilience and home health

Emergency pet-care contact card

A minimal printable card to flag pets at home and an agreed contact.

Keep the card minimal

Carry a card or leave a suitable home copy stating that animals need care and identifying an agreed contact. You do not need to include diagnoses, your full address, microchip number or access codes. Ask the contact before sharing their details.

Keep it current and practical

The contact should know where private care instructions are kept and how to arrange access. Review the card when circumstances change. It is a prompt for communication, not a guarantee that responders can arrange boarding or legal authority.

Fill in privately and print using your browser’s Print or Share menu, or print blank and write by hand. No submission, account, automated interpretation or website storage. Entries may be restored by your browser; use a private device and clear them afterwards. Printed/downloaded copies can contain sensitive information: keep them secure. Avoid access codes, bank details and unnecessary personal health details.

Evidence and review

Owner information, not diagnosis, an individual dose or personal legal advice. Follow the treating veterinary team’s plan. Legal, medicine-supply and poison-service statements apply only to the labelled jurisdiction; general care principles do not establish local rules.
